Screening for vulnerability to functional deterioration in elderly
The ISAR-HP is a screening tool for frailty in the elderly. Previous research showed that the usual cut-off value (≥ 2) resulted in many patients being unfairly classified as vulnerable. An APOP prospective follow-up study has attempted to optimize the predictive value of the ISAR-HP by applying different cut-off values in older, acutely hospitalised patients. A total of 765 patients aged 70 years and older were followed for 90 days. Increasing the cut-off value to a score of 4 turned out to only slightly improve the ISAR-HP performance. Therefore, caution should be exercised to ensure efficient screening and follow-up interventions. In addition, the consequences of a positive screening should be well considered.
